How would you stage this pressure ulcer?

Photo could be better, but it appears to be an "unable to stage" as it looks like the open area is completely covered by slough or eschar (scab). Until that is debrided, it is impossible to stage a pressure injury like this; it could be either a stage III or IV based on the appearance and location.

So I should see someone about it? With what doctor would you recommend I start? I already was in the hospital last time.

It’s kind of on the side of my butt cheek. I was lying slightly rolled backward to avoid pressure my right greater trochanter.

Have your doctor send you to the local wound center.

Ideally see either a wound care physician or a nurse practitioner who is wound care certified (CWOCN is best).

I was looking at the new staging standards for pressure injuries. He seemed to suggest that if you have slough or esehar (is that more than the white stuff or a scab), it is automatically a stage III. I?ve been debrided twice, but the wound nurses tell me I still am not stage-able. Could I still be a stage II?

I am out of sides. My other side has gotten really red, where it stayed that way even after I was off of it for many hours. Nobody pressed on it to check, but if it lasts that long, is it probably not blanching?

I agree that you need to see a wound specialist. If you have a hard to heal wound you need some diagnostics maybe a MRI to look beyond the surface of your wound see if bone is involved.

It is difficult to take pressure off wounds when you have issues on other body aspects. A good mattress surface helps.

My pressure injury still is not stageable (I’m going to hope the person who said “we don’t know – it could be tunneling under there” was being na?ve and dramatic). Isn’t it true that if the area hurts that’s a good sign and it’s not all dead? Now I have something right below my left greater trochanter that my caregiver says is oozing a little although not open (which I can’t quite picture, but someone should be here today to look).

ETA: the nurse said the one on my right side would be definitely stage III or stage IV underneath the black. She said the one on my left side is stage II.
